To receive discounted rates, interested individuals and businesses may apply for
an account with Designs by Doe by completing this form. You will be contacted
shortly to review your application. A minimum purchase volume of $300 per month
is required to maintain an account and receive terms.

Your credit/debit card number will be kept on file with us, and on the day that you
specify in advance will be billed for $300.00, each month. You may order any of
our products and services at any time throughout the following month and receive
the discounted pricing schedule* on every order. Any amount over the initial $300
will be billed on a statement, and given terms of net 30.
